Miraculous Recovery for a 20-Years-Old Boy from Cerebrospinal Fluid Rhinorrhea
The patient encountered frequent unilateral right nasal watery discharge on coughing and bending forward
Mumbai: A team led by Dr Sanjay Helale, ENT specialist and HOD at Kohinoor Hospital, successfully operated a 20-year-old boy with Cerebrospinal Fluid Rhinorrhea by performing Navigation assisted surgery through nose without cutting on face. Family members on the advice of a Neurosurgeon to show it to an ENT Specialist they met Dr. Sanjay Helale at Kohinoor Hospital wherein Dr Sanjay Helale gave him a timely treatment.
Mohammed Arfad Khan, a 20-year-old boy from Mumbai, since 4 months he was suffering from frequent unilateral right nasal watery discharge on coughing and bending forward. Most often, a runny nose is caused by allergic rhinitis or the common cold but the boy was having a rare condition called Cerebrospinal Fluid (CSF) Rhinorrhea, caused by a leaking of brain fluid out of the nose.
“Arfad” met with an accident and got head injury in 2010 and the fluid was coming from the fracture of bone above eyebrow from the frontal sinus. He had similar symptoms after head injury within 2 months and intermittently in last 10 years but it has increased profusely from last 2 months.
Brain fluid leak is commonly and routinely repaired through nose endoscopically however this particular case was leaking through frontal sinus bone above eyebrows, this area is most inaccessible through nose and Navigation was of great help in locating skull base defect.
On CT MRI examination he was diagnosed case of CSF Rhinorrhea which means brain fluid leaking in the nose which is potentially life threatening condition. In such a challenging case mostly requires external opening of scalp, skull bones…etc.
Neurosurgeon from other hospital suggested that they repair the leak by opening through the skull and lifting the brain, however he seek the opinion of Dr. Sanjay Helale. He recommended navigation assisted surgery through Nose without open brain surgery which is called as ‘Transnasal Endoscopic Navigational Surgery’.

CSF Rhinorrhea is caused by a small tear or hole in the membrane surrounding the brain. It can result from a severe head injury, complications from surgery or high pressure in the skull (intracranial pressure). While performing surgery neurosurgeon Dr. Sunil Kutty was there with us for any emergency. The patient is fine after surgery and can do his daily chores with ease.
